Principle of Cold Therapy

Practical application of cold air therapy device

Laser equipment: including picosecond laser (Star Picosecond), long pulse laser (UP Bang Bomb), Q-switched laser, 2940 Erbium laser, CO2 laser, etc., used to treat pigmented skin, vascular lesions, wrinkles and other problems.

Clinical application of cold air therapy device
(I) Study on the onset time of cold air replacing surface anesthesia
The low temperature of cold air can significantly reduce the skin nerve conduction velocity by affecting the exchange between Ca2+ and Na+ in nerve cells. For every 1℃ drop in skin temperature, the nerve conduction velocity will decrease by 0.4m/s. [2] How long does it take for the pain to disappear after cold air is applied to the skin? Sari et al. [3] provides the answer. In this study, they applied -28℃ cold air to the human body surface continuously, and used a 26G injection needle to apply pain stimulation to the test site every 10 seconds, and recorded the time it took for the needle prick pain to disappear after the cold air was applied. According to the experimental results, the time it took for the pain to disappear in women was 30-90 seconds, with an average of 52.88±1.6 seconds; the time it took for the pain to disappear in men was 30-90 seconds, with an average of 56.34±1.52 seconds; there was no significant difference between the two. From this experiment, it can be seen that continuous blowing of cold air can reduce epidermal pain until a painless effect similar to anesthesia is achieved, with a fast onset and no risk of sensitization, which can greatly improve the comfort during skin treatment.
(II) Application of cold air in laser treatment
1. Christian
Raulin et al. [4] compared the pain and adverse reaction rates of cold air cooling and cold gel cooling in the process of using various lasers. The study covered long-pulse alexandrite laser, pulsed dye laser, and Q-switched laser. After comparison, 100% of the subjects said that they would prefer cold air as a facial epidermal cooling method; compared with gel, 63% of the patients had shorter duration of erythema after cold air cooling, 74% of the subjects had reduced erythema, 70% of the subjects had reduced purpura, and 83% of the subjects had reduced scabs; in addition, due to the reduction of pain and the risk of epidermal thermal damage, the energy of the treatment laser can be increased by 15%-30%. In this experiment, after blowing cold air at -15℃ for 8 seconds, the epidermal temperature can be reduced to 15℃, which significantly reduced the pain of treatment and the incidence of adverse reactions, and can be used as an effective auxiliary method for skin laser treatment.
2.Baerbel Greve et al. [5] compared the split surface with cold air cooling and without cooling during the pulse dye laser treatment of port wine stains. After continuous cold air blowing at -15℃ for 8 seconds, the epidermal temperature dropped to 15℃, and then the pulse dye laser treatment was performed. The results showed that 9 patients (69%) felt that laser treatment accompanied by cold air significantly reduced pain, and the incidence of adverse reactions such as purpura, erythema, edema, hypopigmentation, pigmentation, and scars in the cooling area was significantly reduced. Stefan Hammes et al. [6] also reached a similar conclusion. When pulse dye laser was used to treat facial telangiectasia, the pain score and purpura score were significantly reduced after using cold air cooling. It was most appropriate to reduce the epidermal temperature to 20℃. On the basis of not significantly affecting the vascular treatment effect, it improved the patient's comfort and reduced the occurrence of adverse reactions.
3. When using picosecond fractional laser to treat facial photoaging, cold air can also be used for analgesia. Zhou Chenxi et al. [7] confirmed this through experiments. Before picosecond fractional laser treatment, lidocaine cream was applied to one side of the face, and cold air was used to cool the other side. The results after laser treatment showed that the epidermal temperature on the cold air-cooled side was significantly lower than that of the contralateral lidocaine cream group during the operation and until 30 minutes after the operation. This significantly reduced the pain felt by the subjects during the operation, and the analgesic effect could be maintained after the operation, improving the experience.
(III) Application of cold air in injection treatment
Another application of cold air epidermal cooling in dermatology is to reduce pain in conjunction with injections. Falk G. Bechara et al. [8] and Firas Al-Qarqaz et al. [9] have conducted experiments on this use of cold air and reached similar conclusions. Using cold air for 1 minute before a single injection of botulinum toxin can reduce the pain score from 7.4 to 2.5 points, reducing the pain by 66.2%. This greatly reduces the discomfort of patients during injections and improves their tolerance.
