Medical beauty review | A complete guide to pigmentation: causes, classification, treatment and daily improvement

Facial spots are a worry for many beauty lovers, and the appearance of spots is not limited by age and may bother us at any time. So, how can we effectively remove these annoying spots?

The key is to identify the type and cause of the spots. Only by deeply understanding the nature of the spots can we accurately treat the symptoms and find the most suitable treatment method.

Pigmentation is a pigment disorder skin disease, which is mainly caused by uneven distribution of melanin in the skin or imbalance in the amount of melanin produced by melanocytes, resulting in the darkening of local skin color and the formation of spots. These excess melanin cannot be shed normally with the stratum corneum or excreted from the body through metabolism, so it is deposited in the skin, eventually forming various types of pigmentation.

Common pigmentation spots can be divided into two categories: active pigmentation spots and qualitative pigmentation spots.

Active pigmentation spots: They are difficult to treat and are prone to recurrence. They are characterized by high tyrosinase activity, unstable nature, and are easily affected by external factors such as light and endocrine changes, resulting in changes in color depth. Common active pigmentation spots include chloasma, sun spots, pregnancy spots, and dark spots.

Qualitative pigmentation spots: They are relatively stable in nature, not easily affected by external factors, relatively easy to treat, and have a low chance of recurrence. Common qualitative spots include hereditary spots, freckles, brown-blue nevus and pigmented nevus.

1. Freckles

Freckles are a common spot that is closely related to genetics and sun exposure, so they are also called "sun spots". They often occur on the cheeks and nose and are distributed symmetrically. Typical freckles appear as light brown to yellowish brown spots, ranging in size from a needle tip to a grain of rice, with an uncertain number, a smooth surface, and are mostly round or oval in shape.

2. Blue spots

Brown blue spots are an acquired dermal melanocytosis, which manifests as gray-brown, black-gray or black-brown round or irregular plaques of varying numbers. It mainly occurs near the zygomatic region and temples, so it is also called the "zygomatic mother spot". This type of spot will increase with age and may be aggravated by factors such as sun exposure, pregnancy, and endocrine disorders. Its susceptibility factors include middle-aged and young people, the use of contraceptives, ultraviolet exposure, the use of cosmetics, and genetic factors.

3. Melasma

Melasma is a type of spot with a high incidence rate, which can usually only be faded but difficult to eradicate. It is mostly distributed in a butterfly shape that is symmetrical on both sides, so it is also called the "butterfly spot". It is common on the forehead, brow bone, cheekbone, nose and mouth of young and middle-aged women, and appears as irregular spots or patches of light brown to dark brown. The causes of chloasma are relatively complex and may be related to multiple factors such as ultraviolet radiation, changes in hormone levels, genetic factors, and damaged skin barriers.

4. Café de Chine

Café de Chine, also known as "milk coffee spots", is a hereditary skin disease that usually grows larger and increases in number with age. It is a type of birthmark and may also be a sign of some multi-system diseases (such as neurofibromatosis). Café de Chine is mostly distributed on the face and trunk, and is light brown, yellow-brown or dark brown. The pigment is evenly distributed and can be in the form of flakes or dots. The treatment effect varies greatly from person to person, and it is more likely to relapse.

5. Age spots

Age spots are medically known as seborrheic keratosis. They are a type of pigment formed by the accumulation of free radicals (i.e., lipofuscin pigments) produced during human metabolism on the skin surface. It is not limited to the elderly, and may also occur in other age groups. In the early stage, it manifests as flat papules or patches, with skin color or brown color and a smooth surface. As we age, the plaques will gradually increase in size, forming plaques with clear boundaries, slightly raised surfaces, and brown or black in color.

1. Picosecond laser

Picosecond laser can instantly break pigment particles through ultra-short pulse width, precise wavelength and energy, and shatter larger pigment particles into smaller fragments. These fragments are then excreted from the body through metabolism or cleared by phagocytes, thereby effectively treating skin pigment lesions. Picosecond laser freckle removal usually requires a certain course of treatment, generally 4 to 6 treatments.

2. Intense Pulsed Light

Intense pulsed light can penetrate the skin and is preferentially absorbed by pigment particles and hemoglobin in the blood. Without damaging normal skin, it can coagulate blood vessels, destroy and decompose pigment clusters or pigment cells, thereby effectively treating capillary dilation and pigment spots.

Intense pulsed light is suitable for a variety of epidermal pigment spots, such as freckles, coffee spots, freckle-like nevus, junctional nevus, seborrheic keratosis, etc. When the pigment spots are large in area and dark in color, or when there are multiple pigment spots on the face (such as freckles, sun spots, chloasma, pigmentation, etc.), intense pulsed light can be used as the first choice. The treatment cycle is usually once every 4 weeks, and 6 to 8 times is a complete course of treatment.

Skin Care and Sun Protection

Avoid using "three-no" skin care products that are highly irritating or contain glucocorticoids. Be sure to take sun protection measures when going out to reduce the damage of ultraviolet rays to the skin.

Healthy Diet

Keep a light diet and minimize the intake of photosensitive drugs or foods, which may increase the skin's sensitivity to ultraviolet rays.

Regular work and rest

Maintain a regular work and rest schedule to avoid endocrine disorders caused by excessive stress, which in turn affects nerve function and metabolism.

Relax

Avoid excessive tension, anxiety or emotional excitement, as these emotional fluctuations may lead to endocrine disorders. Keeping a calm mind and relaxing your mood are particularly important for skin health.

Precautions for freckle removal:

The freckle removal process usually takes a long time, so patience is essential. After the operation, you must strictly follow the doctor's advice and pay attention to preventing infection and pigmentation.
