DEFINITION
Definition of enlarged pores
There is no clear and unified definition of enlarged pores in medicine. It usually refers to the enlargement of the pore opening. Enlarged pores are defined as pores with a single surface area greater than 0.2/0.3mm². It is mainly judged by visual examination, and can also be assisted by skin testing equipment.
REASON FOR FORMATION
• Excessive sebum secretion: It is common in young people aged 16-35. The sebaceous glands are active and secrete a lot of sebum to enlarge the pores, which are concentrated in the T zone. Greasy and stimulating food, staying up late, emotional tension, etc. will aggravate the problem.
• Skin aging: Natural aging and photoaging cause the loss of elasticity of the supporting structure around the hair follicles, damage to collagen fibers, and enlarged pores.
• Skin inflammation: Inflammatory skin diseases cause local tissue edema and enlarged pores.
• Coarse vellus hair: The vellus hair in the pores is thick and dark, making the pores obvious.
TREATMENT METHOD
Treatment for enlarged pores
Mild pain, non-invasive, basically tolerated by beauty seekers, no downtime, and relatively low price, including intense pulsed light, hair removal and chemical peeling.
• Intense Pulsed Light: Light waves with a wavelength of 560-1200nm, near-infrared light reaches the dermis, promotes the increase of fibroblast activity, rearrangement of collagen, etc., smooths the skin, shrinks pores, and inhibits sebaceous gland secretion. The effect is weak and requires multiple treatments.
• Chemical Peeling: Fruit acid, salicylic acid, and compound acid are applied to the skin to cause controllable damage to the epidermis or dermis, remove keratin, repair and regenerate, and improve closed comedones and blackheads. To prevent adverse reactions, the peeling is shallow and the effect is slow. Multiple treatments are required to gradually increase the concentration or time.
• Hair Removal: Laser or intense pulsed light energy is absorbed by the melanocytes of the hair follicles, destroying the hair follicle stem cells, making the hair thinner and softer or disappearing. For cases where the pores are too deep and coarse due to obvious pores, it has a visual pore shrinking effect.
There is a certain pain, and anesthetics may be required. Some treatments are invasive, but they are basically tolerated. There is a certain recovery time after surgery. These treatments include microneedles, water light, non-ablative fractional laser, etc.
• Microneedles or water light: It is a mesodermal therapy that opens the dermis channel, supplements nutrition, and stimulates collagen production. Microneedles have strong mechanical stimulation to the skin, and the erythema lasts for a long time; anti-aging ingredients such as hyaluronic acid are used for aging pores, and salicylic acid, botulinum toxin, etc. are used for oily pores.
The pain is strong and anesthetics are required. There is a certain recovery time after surgery. Such as ablative fractional laser, gold microneedle, etc.
• Non-ablative fractional laser: Matrix thermal coagulation zone promotes tissue regeneration and remodeling. Such as: 1565nm fiber fractional laser.
• Ablative fractional laser: Such as erbium fractional laser, carbon dioxide fractional laser, forming a stratum corneum loss thermal damage zone, strong skin remodeling effect, but high recovery period and pigmentation risk.
• Gold microneedle: With radio frequency energy, mechanical and thermal stimulation of the skin, the energy reaches deep layers, activates skin remodeling, inhibits sebaceous glands, short recovery period but strong pain.
CHOICES OF TREATMENT
Choices of treatment for enlarged pores
Choose different levels or combined treatments based on the type of enlarged pores, patient age, skin condition, acceptance, economic and work situation assessment.
• Excessive sebaceous gland secretion type: first use intense pulsed light combined with chemical peeling; for those with high acceptance, use gold microneedle combined with salicylic acid.
• Aging type: peeling with fruit acid or mid-layer composite acid; for those with high acceptance, use microneedle, fractional laser or gold microneedle.
• Inflammatory type: first use anti-inflammatory treatment, then add other treatments.
• Vellus hair type: First use intense pulsed light and 755nm laser for hair removal, and use long pulse width 1064nm laser for poor treatment.
