Lasers with a wavelength of 1064nm belong to a special infrared light band, which has a selective photothermal effect on skin color bases. Lasers with this wavelength can be absorbed by water, hemoglobin and melanin in the skin at the same time, although its absorption rate is lower than that of other wavelengths. However, this wavelength has a strong penetrating ability and can reach a depth of 5-10mm in the skin tissue. By adjusting the energy density and pulse width, its action depth can extend from the epidermis to the dermis, and even to the superficial fat or deep blood vessels. Therefore, 1064nm lasers can provide personalized treatment for pigmentary diseases, vascular diseases and many other skin problems.
Picosecond laser: With a pulse width of picoseconds, it can effectively act on pigment particles through photothermal and photomechanical effects. It has certain therapeutic effects on tattoos, pigment diseases, and facial rejuvenation. Given its advantages in pulse width, picosecond laser is gradually replacing nanosecond laser and is used in the treatment of pigmentary diseases. It can not only improve the treatment effect of pigmentary diseases, but also has the advantages of short treatment cycle and mild adverse reactions for patients who are ineffective with nanosecond laser treatment. Complications such as pain, edema, transient erythema, and scab are milder than those of nanosecond laser.
Long pulse 1064nm laser: with millisecond pulse width, it can cause thermal damage to target cells, and the pulse width parameters are adjustable. The product is applicable to a wide range of clinical applications, and the safety and comfort of treatment are high.
Q-switched laser: It has a nanosecond pulse width and is the preferred choice for treating chloasma, nevus of Ota, brown-blue nevus and other pigmented skin diseases in deeper areas.
According to the distribution depth of melanin in the skin, common pigmentary diseases are classified as follows:
Epidermal type: freckles, freckles, coffee spots, solar lentigo, seborrheic keratosis.
Epidermal-dermal mixed type: chloasma, Baker's nevus, lentigo, post-inflammatory pigmentation.
Dermal type: Ota nevus, Ito nevus, tattoo, cheek brown-blue nevus.
In clinical treatment of pigmentary diseases, laser wavelength is often selected according to the depth and color of the skin lesions. At present, Q-switched laser is mostly used to treat epidermal pigmentation lesions, and has high selectivity for the treatment of freckles, freckle-like nevi, and coffee spots, which can be regarded as an ideal treatment.
1. Treatment of epidermal spots
Comparison: Compared with 532nm short-wavelength laser, 1064nm laser is more suitable for Asians. Short-wavelength laser is prone to cause adverse reactions such as postoperative redness, swelling, bleeding, and post-inflammatory hyperpigmentation (PIH), while 1064nm laser treatment has fewer side effects.
Clinical advantages: For dark-skinned people and patients who are sensitive to postoperative side effects, 1064nm laser is a better choice.
Application example: Q-switched 1064nm Nd:YAG laser has significant therapeutic effects on coffee spots and solar lentigo. The treatment is based on mild erythema, and the treatment is once every 2-3 weeks, for a total of 5-8 times, and the follow-up is 6 months.
2. Mixed pigmentary diseases
Indications: such as melasma, post-inflammatory pigmentation (PIH).
Recommended parameters: large spot and low energy (spot ≥6mm, energy ≤3 J/cm²).
Advantages: It can selectively destroy melanosomes in melanocytes, reduce damage to surrounding tissues, and reduce the risk of PIH.
Research data: In a study of 78 patients with post-acne pigmentation, 70% of the patients had significant improvement in pigmentation after 6 treatments.
3. Dermal pigmentary diseases
Indications: Ota nevus, malar brown-blue nevus.
Treatment characteristics: Since the pigment is located in the deep dermis, the 1064nm laser disintegrates the pigment bodies with a higher energy density to avoid damage to adjacent cells.
End point reaction: The skin in the treatment area turns white, and there is slight bleeding after a few minutes.
Precautions: Avoid scarring caused by excessive energy; small spots and high energy density can be used to treat skin lesions of varying depths.
4. Tattoo removal
Indications: Black and blue tattoos.
Advantages: Picosecond laser breaks pigment particles into smaller particles through short pulse width and high energy density, which are easier to be cleared by macrophages.
Comparison: Compared with traditional Q-switched lasers, picosecond lasers have higher removal efficiency and lower risks of postoperative scars and hypopigmentation.
Personalized parameter adjustment: Select the spot and energy density according to the patient's skin color, skin lesion depth and sensitivity.
Treatment test: Test low energy density in a hidden area before the first treatment.
Postoperative care: Apply cold compress immediately after treatment. If pigmentation occurs, wait until the pigmentation subsides before continuing treatment.
