The International Forum for the Study of Itch (IFSI) describes sensitive skin as the discomfort (such as tingling, burning, pain, itching, tingling, etc.) caused by external stimuli, which normally do not cause similar symptoms and cannot be completely attributed to lesions of other skin diseases. The appearance of the skin of the affected area can be normal or accompanied by erythema; sensitive skin may affect the skin all over the body, but the face is the most common part.

When treating sensitive skin, patient health education, scientific skin care, drug treatment or photoelectric device treatment are usually adopted. However, clinical practice has found that although many patients' symptoms have been alleviated after systematic treatment, the tiny blood vessels of patients with sensitive skin are prone to dilation and accelerated blood flow under certain stimuli, thus causing paroxysmal flushing. This phenomenon is usually difficult to be completely improved by drugs and other means. In recent years, photoelectric therapy has been widely used in the clinical treatment of sensitive skin.

So, how does the common intense pulsed light therapy work in the clinical application of sensitive skin? Is it suitable for the treatment of stable and acute phases?

Intense pulsed light has unique advantages in treating sensitive skin, especially when compared with other photoelectric devices. According to the "Chinese Expert Consensus on the Diagnosis and Treatment of Sensitive Skin" - formulated by the Cosmetology Group of the Dermatology and Venereology Branch of the Chinese Medical Association and other institutions - the clinical manifestations of sensitive skin are divided into mild, moderate and severe, and common symptoms include skin swelling, redness, burning sensation and itching.

In China, about 36% of women have sensitive skin, and among the yellow population, 70% of women have sub-healthy skin. In pursuit of fast and effective skin care, many people choose chemical peels or laser treatments, but excessive peels may cause problems such as skin fragility, barrier damage, dehydration and dryness. In contrast, intense pulsed light can promote skin metabolism through irradiation and stimulate the regeneration of subcutaneous collagen, thereby significantly reducing the redness and itching symptoms of the skin. By repairing the epidermal barrier and improving sensitive skin, intense pulsed light has opened up new opportunities for the photorejuvenation market.

Intense pulsed light, with its wide spectrum characteristics, can achieve multiple effects such as redness reduction, desensitization and skin quality improvement in the treatment of sensitive skin. Intense pulsed light can play a positive role in both the sensitive period and the acute period.

Acute period:

In the treatment of the acute period, intense pulsed light mainly relieves symptoms through soothing and desensitization. Using the principle of selective photothermal therapy, intense pulsed light can effectively act on melanin and hemoglobin, promote the synthesis and rearrangement of collagen, and kill facial parasites. Using a 590nm filter can achieve a better light modulation effect. It is crucial to choose the appropriate treatment parameters according to the patient's skin lesions, skin color, individual reactions and other factors. The treatment principle is usually low energy, long pulse width and multiple pulses to ensure safety and effectiveness. The energy is increased by no more than 2J/cm² each time, and is gradually adjusted according to the patient's tolerance. Usually, treatment is performed once every 3 weeks, and 5 times is a course of treatment. During the treatment, it is best for the patient to have no obvious discomfort or only feel a slight fever. The ideal reaction at the end of the treatment is no obvious reaction or slight redness.

Stable period:

During the stable period, intense pulsed light is mainly used to improve skin quality and close dilated capillaries. The treatment plan can be divided into several steps: first, use the 640nm filter to perform a three-pulse treatment, with a pulse width of 6-8ms, a pulse delay of 50ms, and an energy density of 15J/cm²; then, use the vascular filter to perform a double-pulse treatment, with a pulse width of 4/6ms, a pulse delay of 50ms, and an energy density of 13-15J/cm²; finally, use the 590nm filter to perform a three-pulse treatment, with a pulse width of 6-8ms, a pulse delay of 50ms, and an energy density of 12J/cm². The parameter settings for each step are appropriately adjusted according to the patient's skin condition and response to ensure the best treatment effect.

Summary

When using intense pulsed light to treat sensitive skin, treatment should start with low energy and gradually perform light conditioning to relieve over-irritated skin until its sensitive state is improved. The gradual increase and adjustment of energy requires clinicians to flexibly control according to actual conditions and experience. After the skin condition stabilizes, you can consider combining PDL, PDT, or even ablative fractional laser treatments to block blood vessels and thicken the skin.

At the same time, the causes of sensitive skin are complex and intertwined. Therefore, the care for sensitive skin should start from its causes and choose appropriate treatment methods. In addition to conventional daily skin care products and drug treatments, you can also combine medical beauty treatments to comprehensively improve the skin condition through the dual methods of surface care and deep repair. This treatment model that emphasizes both appearance and mechanism helps to improve patients' overall satisfaction with sensitive skin treatment.
